About
Our interdisciplinary Care Team is comprised of an expert panel of Clinical Managers; Registered and Licensed Practical Nurses; Medical Social Workers; Certified Nursing Assistants; Physical, Occupational, and Speech Therapists from your own community. In addition, our agencies work with a Medical Director who provides assistance in evaluating the care we provide and making recommendations for improvement. Our team empowers individuals to regain their independence in the comfort and safety of home. We are on a mission to reverse or slow down frailty in our elderly population focusing on flexibility, endurance, balance, and strength. This will greatly reduce the risk of falls and hospitalizations.
